In May 2017, Elizabethton High School and T. A. Dugger Junior High School Athletics introduced PlanetHS to allow parents and students to complete athletic pre-participation forms online. This program helps ECS staff and parents to eliminate common errors, improve efficiency and ensure compliance.
This means rising 6th - 12th graders interested in playing sports will need to create a PlanetHS account with their parents and complete all forms online. Both the parent and student will need to create a PlanetHS account to complete and digitally sign the web-forms. To see detailed instructions on linking parent and student accounts, watch this video.
“We hope this will be easier for parents, but it is really going to be safer,” said Angie Peters, Assistant to the Athletic Director. “This is a safety measure, and it helps us ensure we have up-to-date information for each child in one place.”
To create an account, parents of EHS students should text S766 to 69274, and parents of TAD students should text S2751 to 69274. They will receive a link which they will use to create an account and input student information. This can be completed on a smart phone. Greater detail about this process is available here.
Applicants will be asked to upload an image of a current physical form from a physician. A free physical clinic will be held at Elizabethton High School on May 8 during the school day. Students do not need to pre-register for the free physical, but they need to have their PlanetHS account created and activated prior to May 8 if they participate in the free physical clinic.
Current students’ 2016-2017 school year physicals and forms will expire after May 19, 2017. Therefore, this process must be completed on or before May 19 or the student will not be allowed to practice or participate until they have provided a sports physical from their own physician and completed the online forms. Students who enroll in Elizabethton City Schools after this date will be able to create an account and update their information prior to participating in athletic programs.
The site allows users to login and logout as needed to update forms. Once the accounts are created and linked, parents and students will simply update annually. With questions, parents should call the EHS Athletics Office at 423-547-8015 ext. 1549.
